I haven't, but then I'm a few thousand km away from NOSM... It depends on where you live, I suppose, but I've waited one week for a letter to be mailed from one side of my city to the other. Summary: difficult to say! Sorry.

I live only 1.5 hr drive from Sudbury (NOSM East) and I still havent recevied my snail-mail package. Should be soon though if they wish for them to returned by the 8th. Perhaps they allow for a faxed confirmation of the signed document ...?

Spoke with admissions, they said that the form can be faxed back, or scanned and sent via email. Give them a call if you haven't received your package by the end of the week. They said they mailed them out last wednesday

Got an interview at NOSM west.
2nd time applying, 2nd time interviewing (@ NOSM)

GPA: UG Psyc 3.61, Currently finishing Master in Cognitive Science but won't get the GPA boost until next year
Context: Born and raised in Sault Ste Marie (22 years). Moved to outside suburb of Ottawa for grad school.
ABS: go girls mentor for big sisters, frosh leader for undergrads, over 300 hrs of hospital volunteering (ICU, information, patient porter, eye institute), daycare volunteer, student council executive, member of several research organizations in development psychology, etc.

GPA: it's 'officially' low...only my 4th year courses were counted ( 2nd degree ) which were all essay/project based. first 3 years of nursing/medical sciences i had a 3.78. previous massage therapy diploma maintained an A average...

Context: well, i am over 40 so have had some time to live in different places. 9 years in rural BC and 9 years in the 'knife. also many work and volunteer op's in rural and remote places but not sure if that counts in context. grew up in t.o. speak french well

ABS: many self directed experiences including starting 2 massage therapy practices in above communities, establishing a music program in rural nfld, creating a CD ( performing musician ) now nursing on an LTC unit while parenting a toddler....
