INTRODUCTION TO THE PLAB:
Professional and Linguistic Assessment Board (PLAB) Exam to practice Medicine in the United Kingdom
The General Medical Council (GMC) is the statutory body responsible for the regulation and standards of the medical profession in the United Kingdom. In order to carry out any sort of medical work or training, a doctor must first obtain registration with the GMC.
Overseas doctors who are seeking limited registration must first pass, or be exempted from the Professional and Linguistic Assessment Board (PLAB) test conducted by the GMC.
PLAB PART 1
PLAB Part-1 is a three hour written exam in an Extended Matching Questions (EMQ) format. The emphasis is on clinical management at the Senior House Officer (SHO) level. The Exam consists of 200 questions in Extended Matching Question (EMQ) and Single Best Answer (SBA) format.
PLAB Part 1 is conducted in the UK as well as in some other countries, currently Australia, Bulgaria, Dubai, Egypt, India, New Zealand, Nigeria, Pakistan, Russia, South Africa, Sri Lanka and West Indies.
On the PLAB Part 1 test, the following four groups of skills will be tested in approximately equal proportions:
a. Diagnosis:
Given the important facts about a patient (such as age, sex, nature of presenting symptoms, duration of symptoms) you are asked to select the most likely diagnosis from a range of possibilities.
b. Investigations:
This may refer to the selection or the interpretation of diagnostic tests. Given the important facts about a patient, you will be asked to select the investigation which is most likely to provide the key to the diagnosis. Alternatively, you may be given the findings of investigations and asked to relate these to a patient's condition or to choose the most appropriate next course of action.
c. Management:
Given the important facts about a patient's condition, you will be asked to choose from a range of possibilities the most suitable course of treatment. In the case of medical treatments you will be asked to choose the correct drug therapy and will be expected to know about side effects.
d. Others:
i. Explanation of disease process: The natural history of disease will be tested with reference to basic physiology and pathology.
ii. Legal/ethical : You are expected to know the major legal and ethical principles set out in the GMC publication Duties of a Doctor.
iii. Practice of evidence based medicine: Questions on diagnosis, investigations and management may draw upon recent evidence published in peer-reviewed journals. In addition, there may be questions on the principles and practice of evidence-based medicine.
iv. Understanding of epidemiology: You may be tested on the principles of epidemiology, and on the prevalence of important diseases in the UK.
v. Health promotion: The prevention of disease through health promotion and knowledge of risk factors.
vi. Awareness of multicultural society: You may be tested on your appreciation of the impact on the practice of medicine of the health beliefs and cultural values of the major cultural groups represented in the UK population.
vii. Application of scientific understanding to medicine
PLAB PART 2
PLAB Part-2 is in the form of a 14 station Objective Structured Clinical Examination (OSCE). Candidates become eligible to take PLAB Part-2 only after passing PLAB Part-1 and must do so within two years of the first exam. This OSCE exam tests an individual’s clinical and communications skills and the ability to perform certain medical procedures. PLAB Part 2 is only offered at a few locations within the UK.
PLAB part 2 exam structure
The PLAB part 2 exam is based on clinical assessment in the form of OSCE, i.e. Objective structured clinical examination. There are 14 stations which last 5 minutes each with 1 minute between stations to read the questions and to plan for the next station.
There may be 1 rest station and 1 pilot station, all together 16 stations. Each station requires you to undertake a particular task: like clinical examination, history taking and appropriate diagnosis, case management (discussion with examiner), counseling a patient and some will involve demonstrating a procedure on an anatomical model (manikins) i.e. Suturing the wound, blood sampling, IV cannulation, PV examination, Cervical smear sampling, PR examination, male bladder catheterisation, Otoscopy, Fundoscopy, etc. CPR (adult, child and rarely infant) and Blood Pressure measurement are compulsory.
The examiner will be closely observing you while doing the tasks. This test is to test your clinical and communication skills. It is designed in such a way that examiner can observe you putting these skills into practice.
